If so, after making your appointment online, please look out for the link included in the email confirmation of your appointment and fill out the required admittance forms. This link will also be included in both the email and SMS reminders sent the day before your appointment. If you are unable to fill out these on-line forms you will need to arrive 15-20 minutes prior to your appointment to allow time to fill out the necessary paperwork.
